Short, in plain language, because a policy nobody reads protects nobody.
Your responses are anonymous by default. When you complete the readiness self-assessment we record your item responses, your domain scores, and a brief facility profile: hospital type, state, approximate annual births, NICU level, and whether a bereavement coordinator is on staff.
We do not record your hospital's name, your name, your location, or any contact information as part of that submission. The hospital name and unit you enter for your own on-screen report stay in your browser and are not transmitted.
Anonymous responses are used for benchmarking and field research. Findings are published only in aggregate, in a form that cannot identify a facility.
Requesting the Leadership Brief and benchmark comparison is optional and separate. If you provide your name, role, work email, and facility, we use that information for two purposes only: to send you what you asked for, and to follow up with you directly about your results.
We do not sell, rent, trade, license, or share your contact information with any vendor, manufacturer, sponsor, advertiser, data broker, or third party. Not for money, not for partnership, not ever. You will not receive marketing from any other organization because you used this tool.
Reply to any message from us and ask to be removed, and we will delete your contact information. Ask us what we hold about you and we will tell you.
The self-assessment collects no patient information of any kind and is not a clinical system. Do not enter patient details into any field. Bereavement Ready is not a covered entity and nothing on this site should be used to transmit protected health information.
This site is hosted on Netlify. Form submissions are processed through Netlify Forms and a Google Apps Script endpoint under Bereavement Ready's control. Standard server logs may record IP addresses for security purposes. We do not use advertising trackers or sell analytics data.
