  1. Open this page in Safari. Chrome on iPhone cannot install it.
  2. Tap the Share button at the bottom of the screen, the square with an arrow pointing up.
  3. Scroll down and tap Add to Home Screen.
  4. Tap Add. The icon appears with your other apps.
On privacy. Responses are not tied to a named hospital unless you enter one. Scores are stored on your device while you work. Nothing is submitted until you complete the assessment and choose to request the Leadership Brief. Questions about how responses are handled can go to hello@bereavementstandard.org.
