Bereavement Ready™Readiness Assessment · Form v1.3
Eight domains. Twenty-nine questions. One honest baseline.
A structured baseline for pregnancy and infant loss care, about fifteen minutes. At the end you receive an indicative readiness reading and a one-page Leadership Brief written in the language leadership decides on.

Each question is scored 0, 1, or 2 against evidence you could show a colleague. The standards belong to the bodies that publish them. This measures readiness against them.
Your answers stay confidential. We record your answers, your role, your hospital and unit, and a brief facility profile (country and region, hospital type, annual births, neonatal unit level, and whether a bereavement lead is in post). Results are published only in aggregate, never by hospital, and are never shared. Your own name stays on this device unless you choose to save your results at the end. How we use this
Recorded with your results so your hospital will not need to retake the assessment if it asks Bereavement Ready for help. It is never published or shared, and no result is ever reported by hospital name.

Your benchmark comparison
Across the units assessed so far, bedside practice scores well above the infrastructure around it: written policy, documentation, and review. A low infrastructure score is the expected pattern, not an outlier. It reflects what the institution has built around your team, not how your team cares for families.
Benchmark as of September 22, 2026. Units chose to take part, so this is a self-selected sample, not a random one.

Save your results and compare with your team
Leave your work email and a copy of this report comes to you, so it is not lost when you close the tab.
Then ask two colleagues on your unit to take it with your team code. Teams rarely rate their own unit the same way, and where you disagree is often the most useful finding. Once they finish, Bereavement Ready sends you a side-by-side by domain. It never shows who answered what.
Used only to send what you asked for and, if you ticked the box, to follow up about your results. Never sold or shared. Privacy
More ways to use your results
A score is hard to take to a CNO. A one-page brief written in the terms leadership decides on is not. Your scores are already filled in.
The Champion Kit gives you talking points and a one-page ask for your manager, your CNO, or shared governance. The Business Case Model puts a number on the cost your facility already carries in turnover and patient experience exposure. It uses US hospital costs.
We go through your three lowest domains together, name what to build first, second, and third, and where each one is likely to meet resistance inside your hospital. You leave with the sequence in writing.
Many units take the sequence and build it themselves. If you want help with the build, we can scope that on the same call.
The email option attaches your results, so there is nothing to retype.
Print or save as PDF opens your print window: choose Save as PDF in the Destination menu. On iPhone or Android, tap Share, then Save to Files.
Act 346 reaches every Louisiana hospital with a maternity or neonatal unit. It names staff training and connecting families to bereavement services. Those two map directly to Domains 03 and 06 of your results above.
A Prioritization Intensive turns this score into a sequenced plan with named owners, a recorded walkthrough, and a one-page summary written for your CNO. No meeting required, delivered within five business days.
